Weight training injuries occur because of improper use of weights. In order to avoid weight training injuries, you should follow the recommendations about which sets of weights instructor advises.
People usually add weights without consulting their instructors because they want to build muscle more quickly. However, this is a common misconception. Weight training requires hard work and a lot of time in order to get the body you want. Do not add weights if you haven't finished your training with a particular set of weights.
